Medical Equipment Procurement: The 6-Step Checklist I Use When Time Is Short

This checklist is for the person who just got told a surgical suite needs a laser surgery system upgrade by Friday. Or a clinic has to replace its defibrillator AED before a state inspection. Or the lab wants a new chemistry analyzer and the board needs an answer this week.

In my role coordinating emergency equipment deployment at a regional healthcare network, I've handled about 40 rush requests in nine years—including same-day turnarounds for surgical teams. Here's the checklist I actually work from. Six steps, no fluff.

Step 1: Name the Clinical Need, Not the Product

This sounds obvious, but it's the most-skipped step. Start with the clinical requirement, not the device category.

"We need a defibrillator AED" is a product sentence. "We need rapid defibrillation capability in the outpatient wing, operated by staff with limited annual training" is a clinical need. Same product category, but the second sentence changes what you evaluate. You'd look for visual prompts, longer battery life, and simple pad-placement guidance.

Same with laser surgery systems. "We need a laser system for ENT procedures" tells me the specialty. "We need a laser that cuts and coagulates in a confined surgical field, in a two-suite outpatient center with six surgeons trained on diode platforms" tells me exactly what matters: tip design, portability, and the real training curve.

Write the need down in two sentences. If you can't, keep digging before you call a vendor.

Step 2: Check Compatibility With What You Already Own

New equipment doesn't exist in a vacuum. It goes into a building with existing power, existing IT, existing storage, and existing people.

I learned this the expensive way. In 2023, we rushed a bedside monitoring system into service—or rather, we tried to. The hardware worked. But our hospital's IT security team required a full risk assessment before any new networked device could join the clinical network. Nobody had budgeted for that. Four extra days, and the "emergency" turned into an extended one.

So run through this before you commit:

  • Power: Voltage, backup supply, outlet locations. Some laser surgery systems need a dedicated circuit.
  • Network: Will this device connect to your EMR or monitoring system? Who has to approve the integration?
  • Physical space: Door widths, elevator clearance, ceiling height. You don't want to unbox a device in the loading bay and discover it doesn't fit the corridor.
  • Staff familiarity: Is this a new workflow or a variant of one your team already knows?

This is also where the "one vendor for everything" argument stalls. A broad portfolio like Stryker's is genuinely useful for standardization. But portfolio breadth isn't the same as knowledge of your specific building. Ask the vendor who owns integration on their side. If they can't name a person, that's a red flag.

Step 3: Ask About Service Response Time—Before You Buy

It took me about five years and 40 equipment emergencies to understand that service responsiveness matters more than vendor polish. A polished sales deck means nothing when the device is down on a Friday afternoon.

If your AED dies before the weekend, what happens Monday? Ask for written answers:

  • What's the mean time to repair (MTTR) for this model?
  • How many field engineers cover our region?
  • Is a loaner available while ours is being repaired?
  • Are replacement parts stocked in-country, or do they ship from overseas? (Overseas is a red flag.)

To be fair, large suppliers like Stryker do have service organizations that cover most of their product lines. That's a real advantage. But service contracts are priced per product line, and response times vary by region. Get specifics in writing, per device. Don't take "we'll take care of you." Take "response within 4 business hours, replacement unit delivered within 48."

In March 2024, a clinic called on a Tuesday needing a replacement AED installed by Thursday evening—state inspection Friday morning. Normal lead time was two weeks. We found a distributor that stocked the unit locally, paid about $200 in expedited delivery on top of the $1,600 base cost, and mounted it with hours to spare. The alternative was a compliance citation on the clinic's record.

That's what response time buys: the difference between a bad inspection report and a quiet Friday.

Step 4: Calculate the Training Burden

A laser surgery system can do remarkable things in the right hands. In the wrong hands, it's an expensive cart that collects dust. So ask upfront:

  • How much training is included in the purchase price?
  • How long does the vendor's educator spend on-site or virtually?
  • What does it cost to train additional staff beyond the first cohort?

Some vendors call a 90-minute webinar "comprehensive training." Others—the ones I trust—acknowledge that their device is specialized and send a clinical educator for a full day. The vendor that says "this isn't plug-and-play" earns more trust from me than the one that promises everyone can learn it in an afternoon.

It's tempting to think training is an operations problem you can solve later. It's not. An untrained team will either misuse the device or stop using it. Both waste the purchase.

Step 5: Total Cost, Not Sticker Price

The quote says the device costs $X. That's not the real cost. Include:

  • Installation and commissioning
  • Training, initial and ongoing
  • Service contracts, warranty extensions, preventive maintenance
  • Consumables and disposables—where the big numbers hide
  • Infrastructure changes: new circuits, IT interfaces, mounting, signage
  • Trade-in or disposal of the old equipment

Quick aside for anyone here because they're evaluating lab purchases: clinical chemistry is the branch of diagnostics that measures chemicals in blood, urine, and other fluids—glucose, electrolytes, enzymes, lipids. The chemistry analyzer is the workhorse. And for analyzers, the reagent and consumable contract often exceeds the purchase price within the first two years.

Same pattern applies to laser systems with single-use fibers, and even to AEDs—batteries and pads expire, and those costs stack up across a fleet.

Ask every vendor for a "cost to own per year, years 1 through 3." If they can't or won't provide one, that's a warning. Don't hold me to this, but I've seen service-contract pricing vary by 30% for identical equipment from different vendors.

Step 6: Plan for the Worst Case

What's the backup plan if this device is down for six weeks?

For an AED, maybe another floor's manual crash cart can cover. For a laser surgery system, maybe you reschedule certain procedures or refer patients to a nearby facility. For a chemistry analyzer, maybe a reference lab partnership with a guaranteed 48-hour turnaround.

We learned this after losing a surgical suite for two weeks in 2022. That's when our procurement policy started requiring a documented backup plan before any equipment purchase. Since then, a device failure is an inconvenience, not a crisis.

The Joint Commission's Environment of Care standards already require accredited hospitals to document medical equipment maintenance and safety programs (jointcommission.org). A backup plan fits naturally into that same documentation.

Smart Hospital Tech: Add These Two Items

One more thing. If you're evaluating that kind of system, everything above still applies. But add two more items to the checklist:

  • Data governance: Where does the video or sensor data go? Who has access? How long is it retained?
  • Security review: Your IT security team should vet the platform before deployment, not after. Build that review into the timeline.

This space is changing fast. As of early 2025, these capabilities were still evolving, so verify current integration requirements directly with the vendor.

Three Mistakes That Waste Money

I've made variations of all three myself. Here's what to avoid.

1. Buying speed over fit. A fast quote doesn't help if the device can't integrate with your infrastructure. The extra week of compatibility work isn't delay—it's insurance.

2. Comparing sticker prices instead of total costs. It's tempting to think you can just compare unit prices. Identical specs from different vendors can produce wildly different long-term costs, mostly because of consumables and service terms.

3. Assuming "one vendor for everything" is the safe choice. Broad portfolios like Stryker's are great for standardization and staff familiarity. But broad doesn't mean best at every specialty. The vendors I trust most are the ones who say, "this is our strength; for the other thing, here's who to check."

That honesty is rare. And it's exactly why I keep working with those people.

The bottom line: urgent procurement doesn't have to be chaotic. Six steps, applied in order, get you to a defensible decision—even when the deadline is real.
